Help - every time one of our employees logs on, a duplicate profile is created. Please help

As soon as one of our employees logs into Domo, she gets a new profile created with different privliges.  How do I stop this?  thank you!

    There is no function in Domo that will create a new user when another user logs on. An Admin must actively create new users in their Admin Settings. Please send an email to support@domo.com with details about the steps that can be taken to replicate this problem. Also specify which user is experiencing the problem when they log on as well as the names of the duplicate users.
