Upload Error - Same Key?

Has anyone had errors upon upload that indicates "An item with the same key has already been added" and what causes this?  I am simply replacing the last data set uploaded.  Will keep trying, and if I figure it out will update

    Make sure you don't have 2 colunns with the same header or columns with blank headers.

    is this an existing dataset your trying to refresh and are you using Domo workbench or something else?

    Thanks for your reply, it is an existing data CSV data set in Workbench.  I did add some columns so could be that it was not an exact replica of the first upload.

    Just opened the source and that is what it was two "city" fields ? Thx so much!
