Error trying to access Domo server in Powerpoint

I have downloaded the Domo plugin for powerpoint but I can't access the server to link them in properly. 

 

When I click "Connect to Domo", I then click "Add" to input the server name, and then the following "What Domo server do you want to connect to?" screen doesn't have an input box and won't proceed to the next step. Screenshot attached.

 

I have deleted the previous plugin and reinstalled, then tried ppt versions with the template set up properly and new blank versions and keep getting the same error. What can I do?

I'm normally moving by the following environment.

 

Windows 7 SP1 64bit
PPT 2013 15.0.4903.1000 32bit
Plug-in 1.1.3.0

 

Plug-in Version
DOMO > Help > About

 

It may be the version of PPT and the one by which a defect is here by a bit.

PPTのバージョンやbitによって不具合があるのかも知れません。
