SEE Pardot Form Handler Information in DOMO?

I've connected Pardot to DOMO, and find right now only have the Forms information from Pardot, but exclude the information about Form Handler. Is there's a way to see the information about Form Handler Performance in DOMO?

    Did you ever figure out an answer to this issue? I am running into the same problem. I thus far am using the Prospect report with a few custom fields to try and indicate what form they used, but it is NEVER 100% accurate. For some reason, Pardot randomly skips prospects.
